New Patient Psychiatric Evaluation with PMHNP

The initial evaluation includes but is not limited to collecting personal and family history, medical history, screening tools, discussing medication recommendations if indicated and formulating a recommended treatment plan.

Computer Based Attention Deficit Hyperactivity Disorder Assessment

This service is for those who do not have a previous ADHD diagnosis and are wanting to explore this for the first time. This will be required in addition to an initial psychiatric evaluation appointment and will include patient work outside of the appointment time. Cost includes the testing fee and follow up appointments to review the results and recommend treatment.

COST: $200.00

Follow-Up Medication Management with PMHNP

This type of appointment includes but is not limited to evaluating response to current medication, discussing possible supplemental support and adjusting the ongoing treatment plan as needed.

COST: $175.00

Extended Medication Management & Therapy with PMHNP

This service includes the same as that of a medication management follow-up plus additional therapy with a nurse practitioner to engage in talk therapy, skills education and general lifestyle modification.

COST: $225.00

50 Minute Counseling Session with LMHC*

These appointments will serve as ongoing therapy sessions in order to work towards treatment goals utilizing talk therapy, skills education, and other family-based therapeutic interventions.

COST: $175.00

This allows providers to be more available to our patients, maintain the highest level of confidentiality and not require a formal diagnosis to seek services. If you have, and want to use, out-of-network benefits with your private insurance company, we can provide you with necessary paperwork to file claims for reimbursement (Superbill) for both appointments and ADHD testing.

We cannot guarantee that you will receive any amount of reimbursement for our services due to the wide variety of insurance policies, procedures and plans. Our services are not covered under the Healthy Indiana Plan (HIP 2.0), Medicare or Medicaid.
